STUDENT ANNUAL EVALUATION FORMS

Confidential, all material is anonymized before being sent to faculty. All forms are required unless otherwise labelled.

Prepare off-line. Copy the text for each section into a text editing program, make your comments inline, then paste finished evaluation into relevant window. Give detailed and constructive (not description) evaluations for each form.

Your views are essential for keeping the program vital and responsive. Faculty and advisors value your impressions and consider your input to be an essential influence on their teaching practice. Your input also assures your fellow students of a high caliber of instruction.

Keep in mind that each workshop and seminar experience is unique and not meant to constitute a fixed curriculum. Considerations for the selection of courses are: diversity of faculty, topics and approaches.
